Rinoplasty

What is rhinoplasty? Rhinoplasty is the surgery in which nose shape and respiration problems, if there are any, are treated. As the nose is in the midface and it affects the aesthetic appearance, rhinoplasty is a frequently practiced operation. How is the rhinoplasty procedure and does it have different types? Rhinoplasty is carried out under general anaesthesia. Technically, it has two types, which are open surgery and closed surgery. In the open surgery, a few mms are incised on the front part of the nose called columella and proceeding from this area, some more parts are incised inside the nose. Thus, it is possible to reach nasal dorsum, nasal tip and the nose cartilage which is called septum. In closed surgery, there is no incision on the columella. There are incisions inside the nose and the same parts are reached through this area. In both the open surgery and closed surgery, the main aim is to treat the structures whose anatomies are distorted such as reducing the excess bones on the dorsum and excess cartilage if there are any, fixing the anatomical problems on the septum cartilage which distorts respiration, bringing the angle of the nose tip to appropriate levels and reshaping the alar base. Is it possible to see the shape of the operated nose before the operation? What is simulation? Some patients want to have an idea of their operated nose before the operation by taking photos. It is possible to do this by the help of two or three-dimensional soft ware programs. It is also possible to make differences in the shape of the nose by making changes on the nose with soft ware programs. The important point is these programs are animation programs and they do not reflect the reality. They may approach the reality a bit. This is because human tissue is a mutli-factor and dynamic structure and it is affected by several factors. Therefore, there may be some differences in the simulations. How long does rhinoplasty last and is it necessary to stay in hospital? How is the post operation process? The average duration of the operation is 1,5-2 hours. Our patients are taken to the hospital room with silicon splints inside their noses, and tapes and casts on their noses. They stay in hospital overnight. The patients will be discharged from the hospital the following day. The patients will use antibiotics, painkillers, ocean water sprays and antihistaminic tablets to prevent sneezing. Approximately 5 days later, the patients are called back to hospital and their silicon splints and nose casts are removed. After the nose splints are removed, new thin tapes are put on their noses which are going to remain there for 5 or 6 days. The patients may take a shower 2 days after the operation. The tapes on the nose are waterproof. When do the swellings recover after the operation and when is the final result seen? Two types of oedema may occur after the rhinoplasty. The first one can be seen on the areas such as supraorbital, cheeks, and forehead in the early stage, sometimes along with bruises and it recovers completely in ten days. The second type is the stage in which the change inside the nose occurs. During the recovery process of the nose, this type of oedema decreases gradually and the nose continuously changes. That’s why the recovery of the nose lasts for one year and the nose takes its ultimate shape in one year.  What should the patient do after rhinoplasty and how often should the patients see the doctor? It is advisable not to wear glasses and avoid traumas on the nose during the three-month period after rhinoplasty. The patients may start walking 1 week after the operation and start exercising 1 month later. It is advised not to smoke. In addition, the patients should not use sprays consisting steroids during the first three months. The doctor sees the patient in 1-6 and 12-month intervals after the operation. Some doctors may see the patients at different times.

Fıller Applıcatıons

Filler application is widespread in cosmetic precedures. How is filler applied? It is a simple application which is done after applying an anaesthetic cream on the area it is going to be applied. How long does the filler applied endure? Although it may vary from person to person, it may endure between 12 and 18 months. Is filler application a painful procedure? Thanks to the anaesthetic cream applied beforehand, it is possible to say that it is not too painful. When can the patient return to his/her normal life ? The patient can return to his / her normal life on the same day. Where can the filler be applied? It is possible to say that there is no part in the body where filler is not applied. The parts on which it is most frequently applied may be listed as lips, chin, cheeks, cheek bones temporal sides, under-eye area and forehead. What should the patients be careful about after the filling application? The patients who have lip filling should avoid hot drinks for a few days. In addition, it is not advised to go to a bathhouse, sauna or to have a hot shower. Massage is not advised unless necessary. To prevent bruises, it is recommended to apply ice in 10-minute intervals during a few hours after the application. Women patients can wear make-up.

Chin Filler

Who is an appropriate candidate for chin filler application? Chin filler is applied to people who have a small chin and inadequate chin position. How is chin filler applied? After waiting for about 20 minutes following the application of local anaesthetic cream on the chin area, chin filling is applied with the help of needles or canullas onto the previously designated area. How long does chin filler application last and when can the patients return to their normal lives? Just after the application of chin filling which lasts for about 5-10 minutes, the patients can return to their normal lives. What should the patients be careful about after chin filler application? After chin filler application, ice is applied to the area on which filling has been applied in order to reduce the risk of bruises. It is advisable to avoid pressure and massage onto the chin. How long does chin filler endure? Is re-application needed? If chin filler is applied with hyaluronic acid fillers, it endures for about 12-18 months. The fillings made up of calcium may endure a little longer. Depending on the degree of the chin deformities, or the patient’s expectations, 1-3 cc of filler will be enough.

LIP FILLER

Who is an appropriate candidate for lip filler? Lip filler is applied to those who are not pleased with the shape of their lips, who want to have bigger lips and who have asymmetrical lips. How is lip filler applied? Anaesthetic cream is applied first and after waiting for the cream to show its effect for about 15-20 minutes, the application is done. The filler substance is applied with long thin needles called cannula. First, lip borders are concretized and contour lines are highlighted. Then, lip volume is increased. It lasts for about 15-20 minutes in total. Is lip filler permanent? Hyaluronic acid fillings are not permanent and they disappear in 12-18 months. What should the patient do after lip filling application? It is advisable not to eat or drink anything during the first few hours after the application. The patients should not put on lipsitck or lip gloss for a few hours. They should avoid too much make-up and pressure.

Nose Fıller

What is nose filler? It is an application done with the products prepared by obtaining hyaluronic acid, which naturally exists in our body, in laboratory environments. Who is an appropriate candidate? Where can it be applied? Nose fillers are applied to the patients whose nose defects are possible to treat without surgery. The patients who have nasal dorsal defects, nasal tip falls and who have slight asymmetry on their noses are the ideal patients for nose filling. How long does nose filling application last and when can the patients return to their normal lives? Local anaesthetic cream is applied on the area nose filler is going to be applied beforehand. After waiting for about 15-20 minutes, the filling application, which last about 5 minutes, is done. The patients can return to their normal lives on the very same day. How long does nose filler endure? Is re-application necessary? Like all hyaluronic acid fillings, its endurance may vary between 8 and18 months. My personal experience supports that nose filler endures for a long time. If similar defects are seen on the nose again, the application might be repeated.

Platelet Rich Plasm ( PRP )

What is PRP ? PRP is an application in which the intense form of various growth factors in the blood cells called thrombocyte are injected under the skin. Who is an appropriate candidate for PRP? It can be applied to people who suffer from hair loss, skin blemish and cracked skin. How is PRP prepared? Does it cause pain during the preparation and application process? PRP is prepared by drawing blood from the patients and centrifuging this blood in special catheters. Pain can be reduced by applying anaesthetic cream. How many sessions is PRP application made up of? PRP treatment is practiced in 3 or 5 sessions depending on the needs of the patients. Session intervals are 3 weeks.

Botulınum Toxin Injection (Botulınum Toksin)

Botulinum toxin injection has been a widespread application which can easily be applied in office conditions. How is botulinum toxin injection applied? It is applied in office conditions by applying an anaesthetic cream, in advance. It lasts about 5 minutes and it does not affect the patients’ daily activities. When will the effect of botulinum toxin start? Its effect is seen in approximately 3 -7 days. How long does the effect of botulinum toxin injection last? Its effect may last between 4-6 months. Where can botulinum toxin be applied ? It can be applied to lots of parts such as forehead, glabella, periorbital area, armpit, and palms. What should the patients pay attention to after the botulinum toxin injection? Apart from avoiding lying on the face and wearing too much make-up, there is nothing else to do. Sometimes patients who have very thin skin may have bruises, so they are advised to apply ice.

Fat Injection (Lipofiling)

In recent years, the popularity of fat injection has increased and there is a lot of research and data in this area. Many cosmetic surgeries can be combined with fat injection. What is fat injection and how is it conducted? Where is fat taken off and where is it injected to? It is an operation through which fat is taken off certain parts of the body and injected to the parts where it is considered necessary and to the parts on which it is wanted to create a beautiful appearance. Hips, breasts, lips and face are particularly the most common parts onto which fat injection is applied. Fat is generally taken off the abdomen, and rarely off the buttocks, inner parts of the knees and thighs. Fat is generally suctioned with cannulas, speacially designed for this operation. It can be conducted under local or general anasthesia. If the area to which fat is going to be injected is too big, general anasthesia can be preferred. What is the purpose of fat injection? How many times should it be repeated? Fat injection makes the area it is injected to bigger. The fat cells which are nourished on the area they are injected to become permanent. Thus, it is possible to make the parts of the body bigger with the help of fat injection. After the first session, depending on the survival of fat cells, it is decided whether additional sessions are going to be needed, or not. What should be done in order to prevent fat loss after the fat injection? Are there a lot of bruises and swelling? Following the surgery, mild bruises and swelling may ocur. It is suggested that there be no trauma and pressure on the parts onto which fat injection is applied. For instance, after the fat injection to the buttocks, it is suggested that the patient sit with a pillow, not wear tights and lie on their side. Smoking may shorten the survival of the fat cells, so the patients are told to quit smoking before and after the surgery.

Excısıng Fat From The Cheeks (Bıchectomy)

It is a surgery technique which is applied to patients who complain aboutthe tickening on the cheeks and who want the puffy appearance on this area todisappear. It is conducted under both general and local anasthesia and it lastsabout 20 minutes. An incision applied inside the buccal mucosa and a part of the cheek fat is taken out. The incision inside the mouth is sutured with absorbable materials. Following the operation; ice application, mouthwash and antibiotic useare necessary.It may swell and there may be mild bruises for a few days. Paying attention to mouth hygiene and consuming liquid food prevents the risk of infection.

Autoplasty (Prominent Ear)

Ears, located on the sides of the head, are important aestheticcomponents and organs looked from different angles. There is a variety ofsurgeries on ear disorders and they are very widespread. What is prominent ear surgery? How is it conducted? Is anaesthesia required? Prominent ear surgery is a surgery in which the disorders whichoccur due to developmental reasons are treated. The surgery is conductedthrough an incision behind the ears and the ears are moved back by suturesbetween ear cartilages and cranial and facial bones. It is conducted underlocal aneasthesia. What age is appropriate for the surgery? Usually, prominent ear surgery is carried out during pre-schoolperiod. 5 or 6 years old of age is suitable for that. However, adults can alsohave prominent ear surgery. What should the patient do after prominent ear surgery? Following the prominent ear surgery, a tape should be used to keepthe ears in their position. This tape should be used during the nights onlyfor six weeks. Medical dressing should be applied to the incision behind theears for one week. Suture is absorable and there is no need to remove it.

Liposuction

What is liposuction surgery? The operation in which fat is suctioned with canullas from the certain parts of the body is called liposuction. The surgery is carried out under general anaesthesia. The patient should stay in hospital for one night after the surgery. Who are the suitable candidates for liposuction? The patients who are not overweight, who have fat in certain parts of their bodies despite exercise and diet and the ones who want to shape their body and who have a quality skin are the suitable candidates for liposuction. The aim of liposuction is not to help the patient lose weight. It is not advisable for the people who are overwight, who have saggings in their skin and who have excessive inner abdominal fat deposits. How is the liposuction surgery process? Depending on the parts of the body it will be applied to, it may last for about 2 hours. It is carried out under general anaesthesia. Tumescent solution consisting of medicine which prevents bleeding and pain is injected to identified parts. After waiting for sometime, the operation is carried out. The patient is clothed a corset after the operation and sent to the hospital room. What is the difference between vaser liposuction, laser liposuction and classical liposuction ? With the improvement of technology, the equipment used in liposuction surgery has also advanced. The techniques in which fat is melted and turned into liquid with radio frequency waves are called vaser. Similarly, a technique in which fat is melted and turned into liquid with laser has come into practice. Their difference from the classical liposuction is fat tissue becomes much softer by the help of this equipment and it becomes easier to suction fat tissues from the certain parts of the body. Thanks to these techniques; bleeding, bruises, pain and irregularities are rarer than the classical technique. It is also possible to suction the fat tissue in a more homogeneous way. Is the result apparent just after the operation? How long is the healing process? Liposuction is not an operation aimed at weight loss. Weight loss will occur because of suctioning fat out of the body but the main aim is to shape the body. Like in all surgeries, fluid retention is inevitable in liposuction, as well. However, this will decrease in 1-2 months gradually. The shape of the body will be apparent in three months. What are the important points to pay attention to after the operation? It is advisable to wear a corset during the six-week period following the operation. In addition to these six weeks, it may be advisable to wear a corset only at nights for another six weeks if need be. The patients should watch their diet and should not gain weight. The patient may have light massages two weeks after the operation and might have a shower two days later. Walking is allowed one week after the operation but doing exercise is allowed one month later.    

Abdominoplasty(Tummy Tuck)

How is abdominaplasty conducted? Abdominaplasty is a type of surgery in which the sagging abdominal skin of the patients who have saggings in their abdominal skin because of various reasons is excised and a tighter abdomine is formed. The sagging in the abdominal skin is excised together with the fat tissue under it and the abdomine is reformed by replacing the remaining upper skin with the old skin. It can sometimes be combined with liposuction in order to make the abdominal skin thinner. Who are suitable candidates for abdominaplasty? People who experience saggings in their abdominal skin after losing weight or giving birth and the ones who experience both aesthetics and hygiene problems are the patients who apply for abdominaplasty. The patients who have saggings in their abdomine and who are overweight at the same time are advised to lose weight first. The patients who smoke are advised to quit smoking 3 weeks before and after the surgery. The ones who are planning pregnancy are told to postpone the surgery until after pregnancy because of the possibility of saggings and other skin problems are likely to arise during pregnancy. How does abdominaplasy proces progress and how long is the recovery process? Abdominaplasty lasts for nearly 2-3 hours. It is carried out under general anaesthesia and the patient needs to stay in hospital for 1 or 2 nights. The patient is called for check one week after the surgery and her medical dressing is changed. The patient wears a corset during 1 month. Where is the scar after abdominaplasty? As the sagging skin will be excised and sutured again, a bow-like scar remains in the lower part of the abdomine to be covered inside the bikini. It is hard to estimate the quality and size of the scar, but by using sunscreen and scar healing cream it may be reduced. With which operations can abdominaplasty be combined? During abdominaplasty, generally liposuction is applied. Sometimes, fat injection can also be applied. The patients who lose a lot of weight and have saggings on their back can be applied 360 degree thightening operation which is called “belt lipectomy”.
